Options Counseling
We can offer information and resources to help meet people's long-term care needs. Options counselors also discuss factors to consider when making long-term care decisions. These topics might include, but are not limited to housing, home care, home-delivered meals, personal response systems, and medication management.
The Aging and Disability Resource Center (ADRC) is the place to go to learn about Long Term Care Programs, including eligibility requirements, and to start the application process. We perform the Wisconsin Adult Long-term Care Functional Screen, which is used to collect information about your ability to complete daily activities of living and how health issues affect your abilities. The functional screen is one determinant of eligibility and gets your application started.
