Transportation for Seniors and Individuals with Disabilities
We partner with local transportation providers to offer discounted rides for eligible individuals. These rides help seniors and adults with disabilities get to scheduled medical appointments. Transportation for shopping, social outings, and personal errands may also be available, depending on funding. Call 715-726-7777 for questions.
- Cost: $5 one-way copay
- Long-distance trips (over 100 miles): Additional $10 surcharge
Note: We can also provide rides for individuals who have a Non-Emergency Medical Transportation (NEMT) benefit but are unable to use it for their trip—for example, when the ride is not for a medical appointment.
Eligibility Requirements
- You must meet all of the following criteria:
- Be age 60 or older, or an adult with a temporary or permanent disability
- Live at home (not in a care facility)
- Have no access to other transportation options, including NEMT
- Have no other funding sources available for transportation (including NEMT)
- Not eligible for Medicaid or BadgerCare, which would provide NEMT as a covered benefit
NEMT Transportation for Forward Health Members
NEMT is a benefit for most Wisconsin Medicaid and BadgerCare Plus members and isn't affiliated with our Aging and Disability Resource Center (ADRC) department. Individuals who are eligible for medical assistance, or could be eligible if they applied, cannot use Chippewa County's discounted transportation program.
- To find out if you already have this NEMT transportation benefit, call Forward Health Member Services at 800-362-3002.
- To find out if you could be eligible, call the Great Rivers Consortium at 888-283-0012 to apply for healthcare programs.
- To schedule a ride with NEMT, call 2 business days ahead at 866-907-1493, Monday through Friday, from 1 am to 6 pm, or on the NEMT website.
Vehicle Modification
This grant provides financial assistance to individuals with disabilities and / or their caregivers who require vehicle modifications to improve accessibility. It is funded through Section 85.21 state funding allocated to the ADRC to support the transportation needs of residents in Chippewa County. Some examples may include wheelchair lifts or ramps, hand controls, swivel seats, lowered floors, transfer seats, a wheelchair restraint system, pedal extension, or a steering aid. Call 715-726-7777 for questions.
