Children's Long-Term Support Waiver
The Children's Long-Term Support (CLTS) Waiver Program provides individualized supports and services to help children with disabilities grow and thrive while remaining in their home or foster care setting.
This Medicaid-based program is available for children and youth (birth through age 21) who have:
- Development Disabilities
- Physical Disabilities
- Severe Emotional Disturbances
Through CLTS, your child and family may receive a range of supports tailored to your goals, strengths, and needs. The services are determined using a person-centered approach called Deciding Together - which puts your family at the center of decision-making. We work with families to develop Individualized Service Plans that will include the services and supports that are needed to address the child and family's unique needs.

To make a referral, please call 715-726-7900 or visit the Children's Resource Center. The Children's Resource Center is a division of Chippewa County Public Health. They are the single point of entry for answering questions, providing information, resources, and outside referrals.
Children's Community Options Program
Children enrolled in CLTS may also qualify for Children's Community Options Program (CCOP), which can cover supports not funded by CLTS, Medicaid, or private insurance.
